It is a chronic inflammatory disease where the immune system attacks the joint and connective tissues connecting the spine, which, if left untreated, causes the vertebrae to fuse.
Hernia pain increases with movement and standing, and resolves with rest. Conversely, Ankylosing Spondylitis (Inflammatory rheumatism) pain increases with rest (while lying down at night), one wakes up with a stiff lower back in the morning, and the pain decreases with movement/exercise.
Although the exact cause of the disease is unknown, genetic predisposition is very strong. The HLA-B27 gene is positive in the vast majority of patients. The disease usually appears in young men between the ages of 15-30.
It does not only affect the lower back and back. It can be accompanied by recurrent redness and pain in the eye (Uveitis), heel spurs (Achilles tendinitis), restriction in the rib cage, and intestinal inflammations (Crohn's, etc.).
A detailed rheumatological examination is conducted, looking at blood inflammation levels (CRP) and the HLA-B27 gene test. To detect inflammation in the pelvic joints (sacroiliitis) at an early stage, a Pelvic MRI is performed.
Nonsteroidal anti-inflammatory drugs (NSAIDs) are used to stop the inflammation. In advanced cases, targeted immunosuppressive 'Biological Agent' injections are prescribed. Regular daily stretching exercises and swimming are an inseparable part of the treatment.
