It is a syndrome where the immune system, which normally defends the body against infections, mounts an exaggerated and uncoordinated cascade reaction, causing catastrophic damage to healthy host organs (such as kidneys, heart, and lungs) alongside targeting the pathogen.
Even localized infections such as uncomplicated urinary tract infections, pneumonia, cutaneous lesions, or a ruptured appendix can precipitate bacteremia (presence of bacteria in the blood), subsequently triggering the septic cascade.
It is the most severe and highly lethal manifestation of sepsis. Systemic inflammation causes massive vasodilation, resulting in a dramatic collapse of arterial blood pressure (hypotension) that deprives vital organs of perfusion, culminating in rapid cellular necrosis.
Clinical markers include extreme hyperthermia or hypothermia, violent rigors, marked tachycardia (palpitations), tachypnea (rapid and labored breathing), anuria (cessation of urine output), mottled skin (cyanosis), and profound altered mental status.
The geriatric population (over 65 years), neonates under 1 year of age, patients with oncological or diabetic morbidities, and immunosuppressed individuals (e.g., solid organ transplant recipients) are exceptionally vulnerable to septic episodes.
The management of sepsis adheres to the 'Golden Hour' paradigm. If intravenous broad-spectrum antibiotics and aggressive fluid resuscitation are not administered within the first hour of diagnosis, mortality risk escalates exponentially with each subsequent hour.
