Cerebrovascular diseases represent a spectrum of neurological morbidities resulting from the disruption of cerebral blood flow secondary to vascular occlusion, stenosis, or hemorrhage, ultimately leading to localized cerebral infarction. This category encompasses major events such as acute ischemic strokes and Transient Ischemic Attacks (TIAs). In our Neurology and Interventional Radiology departments, irreversible brain damage is actively mitigated through emergency intravenous thrombolytic therapies and endovascular mechanical thrombectomy or stenting procedures.

It is the collective nomenclature for conditions characterized by inadequate cerebral oxygenation and subsequent neuronal necrosis, precipitated by thromboembolic occlusion, vascular rupture (hemorrhage), or structural aberrations (such as aneurysms) within the cerebral vasculature.

It primarily manifests through two mechanisms. Either a migratory thrombus from the carotid arteries or heart occludes a cerebral vessel (Ischemic Stroke), or a compromised vessel ruptures due to chronic hypertension, extravasating blood into the brain parenchyma (Hemorrhagic Stroke). Both etiologies result in acute focal neurological deficits within seconds.

Colloquially referred to as a 'mini-stroke'. A thrombus temporarily occludes a vessel, inducing transient symptoms such as unilateral paresthesia or dysarthria, but the clot undergoes spontaneous endogenous lysis within minutes to hours. Despite the resolution of symptoms, it serves as the most critical prodrome of an impending massive, irreversible stroke, demanding immediate clinical intervention.

Uncontrolled hypertension, Diabetes Mellitus, chronic tobacco consumption, hypercholesterolemia, Atrial Fibrillation (cardiac arrhythmia), and profound obesity represent the most significant pro-atherogenic risk factors.

Unilateral facial paresis or drooping (Face), unilateral motor weakness or hemiparesis in the upper extremities (Arms), and slurred or incomprehensible articulation (Speech). The presence of even a single criterion dictates that there is no Time (Time) to lose, and emergency medical services must be activated immediately.

In acute ischemic stroke, patients presenting within the strict 4.5-hour therapeutic window are administered intravenous thrombolytics (clot-busters). For large vessel occlusions, neuro-interventional endovascular thrombectomy is performed to mechanically retrieve the clot. In hemorrhagic events, strict blood pressure control is initiated, or neurosurgical hematoma evacuation is considered. Post-acute management heavily relies on intensive neuro-rehabilitation.
