General Information on Malaria

Malaria is an acute, life-threatening infection caused by protozoan parasites of the genus Plasmodium, transmitted to humans through the bite of infected female Anopheles mosquitoes. The disease progresses by invading and lysing red blood cells. It is clinically characterized by severe rigors, high-grade fever, and profuse diaphoresis (sweating). What is Malaria?

It is one of the world's most widespread and lethal infectious diseases, caused by a microscopic single-celled parasite called Plasmodium, which invades human erythrocytes (red blood cells), multiplies within them, and subsequently causes them to rupture.

How is it transmitted?

It is transmitted to the bloodstream solely through the bite of female Anopheles mosquitoes harboring this specific parasite. The disease is not spread directly from person to person through casual contact or respiratory droplets. It can only be transmitted anthropogenically via blood transfusion or vertically from mother to fetus through the placenta.

How does a Malarial Paroxysm (Rigors - Fever - Sweating) occur?

When parasitized red blood cells rupture simultaneously en masse, a massive cytokine and toxin storm occurs in the bloodstream, triggering the classic 'Malarial Paroxysms'.
1. Initially, the patient experiences a severe cold stage with violent shivering (rigors) that can make the teeth chatter.
2. Subsequently, the core body temperature spikes abruptly to 40-41°C (high-grade fever).
3. In the final stage, the patient sweats profusely (drenching the bedclothes), the fever subsides, and the patient falls into an exhausted sleep. These paroxysms typically recur every 48 or 72 hours depending on the Plasmodium species.

Why does it cause Anemia and Jaundice?

Because the parasite continuously targets and lyses erythrocytes (red blood cells), the patient rapidly develops profound hemolytic anemia. The massive release of 'Bilirubin' from the destroyed erythrocytes leads to distinct yellowing of the skin and sclerae (icterus/jaundice), accompanied by massive enlargement of the spleen (splenomegaly) and liver (hepatomegaly).

How is it diagnosed?

Immediate blood sampling is mandated for any febrile patient with a recent travel history to endemic regions such as Africa or Asia. The blood sample is smeared on a slide and examined microscopically (Thick and Thin Blood Smear Tests). Direct microscopic visualization of the intra-erythrocytic parasites yields a 100% definitive diagnosis.

What are the therapeutic and prophylactic measures?

Malaria is a fully curable disease if diagnosed promptly and treated aggressively. Depending on the Plasmodium species, potent and specific Antimalarial agents (such as Chloroquine, Artemisinin-based combination therapies, or Mefloquine) are administered orally or intravenously.
Prevention: Prior to traveling to malaria-endemic regions (e.g., Sub-Saharan Africa), it is imperative to initiate Antimalarial Prophylaxis (preventive medication) 1-2 weeks before departure and continue the regimen strictly throughout the duration of the trip.
