It is a severe lower respiratory tract infection where the alveolar sacs, which facilitate gas exchange (oxygenation), become infected and engorged with purulent exudate (inflammatory fluid), rendering respiration and oxygen transfer severely compromised.
Influenza (Flu) is a systemic viral illness that affects the entire body, causing profound myalgia (muscle aches) and systemic symptoms.
Bronchitis involves inflammation localized strictly to the major conducting airways (bronchi); it produces cough but does not penetrate deep into the pulmonary parenchyma.
Pneumonia, however, involves microbes penetrating deeply into the functional spongy tissue (alveoli) of the lungs, obstructing them and filling them with fluid. It is the most severe and life-threatening among the three.
Clinical presentation includes acute-onset teeth-chattering rigors (chills), high fever surging to 39-40°C, complete anorexia, and debilitating fatigue. The most pathognomonic symptom: A sharp, knife-like pain in the chest or back when taking a deep breath or coughing (Pleuritic chest pain). The patient typically expectorates thick, purulent sputum that may be green, rust-colored, or blood-tinged. In advanced cases, severe hypoxemia leads to bluish discoloration of the lips (Cyanosis).
It is a milder form of pneumonia, frequently caused by Mycoplasma pneumoniae bacteria, predominantly observed in young adults (especially in congregate settings like schools or dormitories). The fever is typically low-grade, and the patient does not become bedridden, continuing daily activities (work/school). However, they experience a highly persistent, severe, and irritating dry hacking cough lasting for weeks. It is colloquially termed 'walking pneumonia' because patients remain ambulatory.
During auscultation, instead of normal vesicular breath sounds, the physician hears 'crackles/rales', resembling the sound of bubbles popping underwater. The diagnosis is confirmed with 100% certainty in the emergency department via a PA Chest X-ray, which reveals a prominent white opacity (consolidation/infiltration) in the pulmonary regions that should normally appear air-filled (black).
If the etiology is bacterial (which accounts for the vast majority), the cornerstone of therapy is ANTIBIOTICS. Young, clinically stable patients are managed in outpatient settings with high-dose oral antibiotics and expectorants.
Conversely, patients over 65, those exhibiting dyspnea, hypotension, or oxygen saturations dropping below 90%, MUST BE HOSPITALIZED (or admitted to the ICU) for intravenous antibiotic administration and supplemental oxygen therapy.
Prevention (Vaccination): There is a Pneumococcal Vaccine targeting Streptococcus pneumoniae, the leading bacterial cause of pneumonia. It is imperative for all individuals over 65, as well as those with chronic comorbidities (Asthma/COPD, diabetes, cardiovascular disease), to receive this vaccine at their primary care clinics, as it significantly mitigates mortality risk.
