It is a tear occurring in the thin and sensitive skin layer at the exit of the anus.
The most common cause is chronic constipation and hard, thick stool straining the anus. Severe diarrhea attacks or birth traumas can also cause a tear.
Hemorrhoids are the swelling and bulging of the veins in the anus. A fissure is a direct tear in the skin and causes a much more severe, cutting pain than piles.
A tearing/glass piercing sensation that can last for hours during and after defecation, and seeing bright red fresh blood on the toilet paper.
The patient avoids going to the toilet due to the pain, which increases constipation, and the tear fails to heal and becomes chronic (permanent).
In the acute stage, warm water sitz baths, a high-fiber diet, and muscle-relaxant creams are used. For chronic fissures, Botox injection or surgical muscle cutting (LIS surgery) is applied to relax the anal muscle.
