General Information About Anal Fissure

An anal fissure is a small tear in the delicate mucosa lining the anal canal, usually caused by hard or forceful bowel movements. It presents with severe glass-cut-like pain and fresh bleeding during defecation. In our General Surgery unit, it is treated with medical treatments in the early stage; and with botox or LIS surgery targeting the sphincter muscle in chronic cases.

What is an Anal Fissure?

It is a tear occurring in the thin and sensitive skin layer at the exit of the anus.

What Causes It?

The most common cause is chronic constipation and hard, thick stool straining the anus. Severe diarrhea attacks or birth traumas can also cause a tear.

What is the Difference from Hemorrhoids (Piles)?

Hemorrhoids are the swelling and bulging of the veins in the anus. A fissure is a direct tear in the skin and causes a much more severe, cutting pain than piles.

What are the Symptoms?

A tearing/glass piercing sensation that can last for hours during and after defecation, and seeing bright red fresh blood on the toilet paper.

What Happens if Left Untreated?

The patient avoids going to the toilet due to the pain, which increases constipation, and the tear fails to heal and becomes chronic (permanent).

What are the Treatment Methods?

In the acute stage, warm water sitz baths, a high-fiber diet, and muscle-relaxant creams are used. For chronic fissures, Botox injection or surgical muscle cutting (LIS surgery) is applied to relax the anal muscle.
