General Information About Prostate Inflammation

Prostate Inflammation (Prostatitis) is the inflammation and swelling of the prostate gland, located just below the bladder in men, usually caused by a bacterial infection or sometimes unknown irritations. Unlike benign prostatic hyperplasia (BPH) mostly seen over the age of 50, prostatitis is the most common urological problem in young and middle-aged men under 50. It causes chronic, severe pain in the pelvic area, testicles, and perineum, significantly lowering the quality of life. It is the infection of the prostate gland or chronic inflammation due to neurological/muscular reasons. It is accompanied by severe pelvic (lower abdominal) pain and urinary problems.

Acute Bacterial Prostatitis: Starts suddenly with high fever, chills, and inability to urinate. It is very severe and requires immediate hospital treatment.
Chronic Bacterial Prostatitis: Occurs when bacteria hide inside the prostate and cause mild, recurring infections over months.
Chronic Pelvic Pain Syndrome (Non-Bacterial): The most common type (90% of cases). Despite the absence of microbes, the patient suffers continuous inflammatory pain in the prostate and testicles.

Continuous, dull, and uncomfortable pain in the perineum (between the testicles and the anus), groin, or lower back is the most typical symptom. Burning during urination, frequent urination, a feeling of not fully emptying the bladder, and pain during or after sexual intercourse (ejaculation) are also experienced.

Prostatitis can indirectly impair erection quality (due to pain and stress) and cause premature ejaculation. The bacterial type can be transmitted sexually, but the most common non-bacterial type is definitely not contagious and will not pass to your partner.

A highly sensitive and painful prostate during a rectal examination performed by a urologist confirms the diagnosis. The doctor gently squeezes the prostate (prostate massage) to take a fluid sample from the urinary tract, which is then analyzed in the laboratory for bacteria/inflammatory cells.

The structure (capsule) of the prostate gland is very resistant to drug penetration. Therefore, if microbes are present, prescribed antibiotics must be used continuously for 4 to 6 weeks. In the non-microbial (chronic pelvic pain) type; hot water sitz baths, alpha-blocker pills that relax prostate muscles, painkillers, stress management, and regular prostate massage are used to break the vicious cycle created by the disease.
