General Information About Delusional Disorder

Delusional disorder is a psychiatric illness in which a person holds unshakable and irrational beliefs (delusions) that do not exist in reality but feel entirely real to them. Unlike schizophrenia, hallucinations are not prominent, and the person can continue their daily life. In our Psychiatry department, it is treated with antipsychotic medications and cognitive psychotherapy.

What is Delusional Disorder?

It is when a person blindly believes that a certain idea (being followed, being cheated on, being someone special, etc.) is absolutely true, despite there being no evidence and everyone around them saying otherwise.

What is the Difference from Schizophrenia?

In schizophrenia patients, there is intense hearing of voices (hallucinations), speech impairment, and a severe mental decline. In delusional disorder, the person speaks very logically, goes to work, and looks completely normal from the outside; they only disconnect from reality when that 'single topic' they believe in is brought up.

What are the Types of Delusions?

The most common types include believing one's spouse is cheating (Jealousy), believing the mafia or the state is after them (Persecutory), believing parasites are crawling in their body (Somatic), or believing they are secretly loved by someone very famous (Erotomanic).

Does the Person Know They Are Sick?

No. The most challenging aspect of the disease is the person's lack of insight (egosyntonic). They never accept that what they believe is a 'disease,' so they usually refuse to go to the doctor or are brought in by force.

How Does It Affect Daily Life?

The person can show excellent functionality in matters unrelated to their delusion. However, due to their delusions, they may inflict violence on their spouse (jealousy delusion), take their neighbors to court, or lock up their house like a fortress.

How is it Treated?

Treatment is difficult because the patient resists medication. Antipsychotic drugs are prescribed under the guidance of a psychiatrist who builds a relationship of trust. The aim of therapies is not to refute the patient's belief by calling it 'nonsense,' but to reduce the stress this belief creates in their life.
