Voice Aesthetics (Voice Thinning / Thickening)

About the Treatment Method

Voice aesthetics or Voice Surgery (Phonosurgery) is the process of thinning or thickening the voice through minimal surgery or botox/filler interventions on the larynx and vocal cords, in cases where a person's voice frequency (pitch) does not match their gender, age, or social status, leading to psychological problems. It is applied by Ear, Nose, and Throat specialists in gender transition operations or structural (mutational falsetto) problems.

How is voice thinning (Glottoplasty) done?

The reason the voice sounds deep is that the vocal cords are long and thick. The voice thinning procedure is usually performed under general anesthesia, entering with cameras solely through the mouth without making any external incision on the neck. A portion of the vocal cords is sutured together using lasers or micro-sutures (Wendler Glottoplasty technique), the cords are shortened and made taut, and the voice frequency is pulled to a thin (feminine) tone like a violin string.

How is voice thickening (Thyroplasty) applied?

Conversely, it is done to loosen the vocal cords in men whose voice has remained thin (childish) during puberty or in gender transitions. Mostly, the thyroid cartilage is reached through a small incision from the outside of the neck. The patient is awake, and while they are talking (the depth of the voice is listened to), a small block (titanium/silicone) is placed between the cartilage, and the vocal cords are loosened (Type 3 Thyroplasty).

Can I talk immediately after the surgery?

Especially after voice thinning operations, patients are given "Absolute Voice Rest" (even whispering or coughing is forbidden) for at least 1 week to 10 days for the tissues to fuse (stitches to hold). For the voice color to settle into that desired natural and permanent tone usually requires a voice therapy process (accompanied by a therapist) that can take 3 to 6 months.
