Non-Surgical Treatments of Herniated Disc

About the Treatment Method

A vast majority (more than 90%) of patients diagnosed with a herniated disc (lumbar disc herniation) can be treated with non-surgical (conservative) methods without the need for surgical intervention. Options such as physical therapy and rehabilitation applications, manual therapy, epidural steroid injections (targeted needle treatments), and ozone therapy aim to reduce the edema in the nerve root, stop the pain, and cause the hernia to regress by strengthening the muscles around the spine.

What is an Epidural Steroid Injection (Targeted)?

It is the process of entering exactly the point where the hernia presses on the nerve with a very fine needle under the guidance of a fluoroscopy (X-ray) device and administering a special cortisone and local anesthetic mixture. Its purpose is not to destroy the hernia, but to rapidly stop the pain by "extinguishing" the severe inflammation and edema in the nerve. The procedure takes 15 minutes.

Does physical therapy pull the hernia back?

Traction (pulling), hot/cold applications, and laser therapies used in physical therapy relieve spasms. The personalized exercise programs applied subsequently strengthen the core muscles around the waist almost like a "natural corset", relieving the load on the vertebrae. When the load decreases, the herniated cartilage tissue may shrink (resorb) and regress on its own over time.

In which cases does non-surgical treatment not work?

If the patient has urinary or fecal incontinence, sudden and progressive loss of strength in the leg (drop foot), or sexual dysfunction (Cauda Equina Syndrome), these conditions require emergency surgery, and no time should be wasted with needles or physical therapy.
