Prostate Surgery (TURP and HoLEP)

About the Treatment Method

Benign Prostatic Hyperplasia (BPH) is a condition in which the prostate gland in men enlarges with advancing age, compressing the urinary tract (urethra) passing through it and making urination difficult. In cases where medications are insufficient, closed surgical methods are applied to open the pathway by carving the prostate tissue from the inside. Today, the classic TURP, which scrapes with electrical energy, or the new generation HoLEP technologies, which enucleate (remove the core) with a laser, are used for this procedure.

What is the advantage of HoLEP Laser surgery over classical TURP?

In TURP surgery, the enlarged prostate is hollowed out by burning it piece by piece from the inside. In HoLEP (Holmium Laser) surgery, the enlarged prostate tissue is scratched with a laser in one piece (just like separating the inside of an orange from its peel), stripped from its capsule, and fragmented in the bladder before being removed. In HoLEP, bleeding is close to zero, the risk of the prostate growing back is much lower, and the patient gets rid of the catheter 1 day later.

Is there urinary incontinence or loss of sexuality after surgery?

Closed prostate surgeries performed for BPH do not damage the nerves that provide erection; therefore, the patient's sexual potency is not affected. Since the sphincter (the muscle that holds urine) is also preserved, the risk of permanent urinary incontinence is less than 1%. Only temporary sudden urges to urinate may be experienced for the first few weeks.

What is the absence of semen (Dry Orgasm) after surgery?

This is an expected and natural consequence of these surgeries. Because the inner parts of the prostate are hollowed out, that valve system between the urinary tract and the bladder expands. The patient experiences the sensation of orgasm normally during intercourse, but the produced semen squirts not outward (to the tip of the penis) but into the cavity (into the bladder) and is later excreted with urine. It has no harm to health or pleasure; it only makes it difficult to have children naturally.
