Both Breathing and Aesthetics
If you have a blockage (curvature) in your nose and you only have external aesthetics done (or vice versa, if you only correct the inside), the statics (load-bearing walls) of the nose may weaken, and your breathing problem may get worse over the years. Correcting the curved cartilages inside and using those removed cartilages as support for external aesthetics (to lift the nasal tip) is the most perfect approach for the surgeon.
Your ENT or Plastic Surgery specialist decides according to the severity of the aesthetic and functional defect. If major changes are to be made to the nasal tip, "Open Septorhinoplasty" (with a small incision under the nasal tip) is generally preferred. The "Closed" technique can also be used for minimal problems.
The process is exactly the same as just having an aesthetic nose surgery (rhinoplasty). Having both procedures done at the same time does not create extra pain or a longer convalescence period for the patient. On the 7th day after surgery, splints are removed, and swelling slowly begins to subside.
