A high level of Anti-TPO in the blood indicates that the immune system is damaging thyroid tissue (an autoimmune process). It is most frequently found to be elevated in Hashimoto's thyroiditis.
It is requested alongside TSH and T4 tests when symptoms of thyroid dysfunction are observed, such as unexplained weight gain or loss, persistent fatigue, hair loss, and intolerance to cold or heat.
Fasting is not strictly necessary for the Anti-TPO test. However, since your doctor will likely request other thyroid hormones (like TSH) concurrently, it is generally recommended to have your blood drawn on an empty stomach in the morning. A physician should be consulted regarding the use of thyroid medications on the day of the test.
