Leukocyte Esterase

General Information About the Test

Leukocyte Esterase is an enzyme found inside white blood cells (leukocytes). This test is a chemical parameter checked in a urinalysis (Complete Urinalysis), not in the blood. A positive leukocyte esterase in the urine indicates the presence of white blood cells in the urinary tract or kidneys, and therefore, that the body is fighting an infection (Urinary Tract Infection) in that area.

Is a positive result definitive for a urinary tract infection (UTI)?

It is generally one of the strongest indicators of a urinary tract infection (especially if it is positive along with the nitrite test). However, for a definitive diagnosis and appropriate antibiotic treatment, a urine culture must be performed to determine the type of bacteria.

Can it be positive even without bacteria?

Yes. Even without bacteria, bladder stones, kidney stones, vaginal discharge mixing into the urine, or certain kidney inflammations can cause leukocytes to pass into the urine, resulting in a positive test.

How should the urine sample be provided?

For accurate results, the first urine of the morning should be preferred. To prevent contamination of the urine container with foreign matter, the genital area should be cleaned, and the "midstream" portion of the urine (allowing the first and last drops to pass outside) should be collected in the container.
