An elevated PDW value indicates that very large and very small platelets are present together in the blood (size inequality). In situations such as bleeding, severe infection, or bone marrow overactivity, the bone marrow rapidly releases young and large platelets into the blood, thereby increasing this value.
A low PDW indicates that the sizes of all platelets in the blood are very close to each other (uniform in size). It is generally not a sign of disease on its own, but can be seen following viral diseases that suppress the bone marrow.
